Advanced Services

Everything in the Basic Service plus a fully typed, professionally formatted OMT Access Plan created from your completed intake form.
I take your information, rewrite it clearly and cleanly, and prepare up to four printed copies for your trusted contacts — so each person has exactly what they need, when they need it.
Your Access Plan is organized, easy to read, and designed to reduce confusion during moments when life doesn’t pause.

$10 per Notarized Signature
Family Signing Session: $75
Emergency Contact / Caregiver Signing Visit: $60
Mobile Notary Travel Fee: $25 (Plano)
Outside Plano (Frisco, Allen, Richardson): $35–$45
